The silver Medal (diameter of 4cm) is placed in a black box (55/75/3cm) The face of the Medal includes Polish and American eagles and an inscription 'Wejście Polski do NATO' (Polish NATO accession) The reverse includes an inscription 'North Atlantic Treaty Organisation 1999' against the background of NATO emblems
